Devotees (Visitor) Registration Form

Please read carefully before signing up to visit the temple.

Please don't sign up if you are not sure about visiting the temple and help others use the opportunity to get darshan.

1. Number of devotees (visitors) for darshan is limited to 25 for each 1 hour session.
2. All devotees (visitors) must bring their own masks and wear them at all times inside the temple.
3. Social distancing of minimum 6 feet must be practiced inside the temple all the times.
4. No outside flowers or fruits or prasadams to be brought to the temple.
5. Devotees can continue to sponsor for Online Poojas/archanas/abhishekams by visiting the temple website at www.sribalajivediccenter.org
6. If experiencing fever, cough, cold, sore throat, chills and body ache within last 48 hours, then please do not visit the Temple.
7. If you have traveled in past two weeks then please do not visit the temple.
